Who is Yuzu Health? Yuzu Health is a tech-enabled third-party administrator for innovative health plans. We manage claims, payments, eligibility, and benefits for these plans.
How do I submit a claim? How you submit a claim differs based on the patient’s plan information. Please check the back of their member ID card for the correct claims address.
How do I get paid? Yuzu Health will remit payment to you for claims post adjudication. If you are a direct primary care provider, Yuzu Health will remit your fees to you on a monthly basis in accordance with eligibility.
How do I find eligibility?
  • Yuzu Health provides Direct Primary Care practices with a login to the secure Yuzu Health portal. From there, DPCs can download an eligibility report to get up-to-date eligibility each month. This report is compatible with Hint, so you can upload it directly into your system!
  • If you do not know your portal log-in, reach out to support@yuzu.health.
How do I give other team members access to the Yuzu Health portal? At least member of each DPC team will be considered “Admin.” These admin are able to add additional team members to the Yuzu Health portal
